Hey Shelby not sure if you know or want to see more Form covers.
To avoid turmoiling about the covers of Form magazines you provoke yourself with. No browse capability or sensual physical presence, but there is a great archive available with no boundaries, just for us. For example the two issues you posted are under the 1968 heading here:
